Our Services -

We serve as a geriatric care manager who can guide you as you care for your senior. We evaluate each client’s needs with a comprehensive assessment, then build a specific plan for now and for the future. We help you distinguish symptoms of concern from typical signs of aging. While we’re expert crisis managers, our goal is to establish a relationship with you before your needs change.

Your manager will act as your human hub who gathers information and—with the proper consent—disseminates it to the people who need it. Think: physicians and therapists. Rehab and home care providers. Nurses and senior community staff. We’ll communicate with hospitals about medication and make sure those meds make it home. Think of us as the spoke of a wheel, coordinating all aspects of your care to ensure everything runs smoothly.

We also represent your loved one’s preferences and questions during appointments, insurance phone calls, and family meetings. We’re impartial and nonjudgmental, honoring the family roles you already have in place.

We provide consistent home visits (wherever home may be), calls, and check-ins with your loved one. We execute the medical plan of care and guide them in defining a daily routine that includes healthy choices and plenty of joy-filled, meaningful moments.

We stand by ready to help with whatever you need. We can schedule appointments, arrange transportation, accompany your loved one to the appointment, and give you a written report. Or we can just schedule the appointment and send you a reminder. It’s up to you. We’ll make sure you get the documents and forms you need, then fill them out for you if it helps.

We can even help solve complex issues, such as medication management, by recommending various solutions geared to their abilities and preferences. Does your loved one need help with organization, grocery shopping or getting bills paid on time? We’re here for that, too.

We like to say we’re experts in the small things that can make a big difference. We keep an eagle eye on behind-the-scenes details that are easy to miss when you’re thinking big-picture.

We watch for kinks in electronic health records that make it hard for one provider to access another provider’s notes. We monitor prescriptions so your loved one doesn’t accidentally take old meds when they happen to auto-refill. These details save you money on repeat hospitalizations, duplicate billings, and duplicate tests.

Decades in senior care have made us experts in a lot of things that may be giving you headaches today: access to care options; disability insurance and coverage questions; communication with healthcare providers; legal documents like powers of attorney, releases of information and DNRs. The AgeWell Options Purple Binder is a collection of hard-copy and digital resources and records that stay with each AgeWell client (your family member). Inside you’ll find the assessment, advanced directives, personalized appointment calendars, daily schedules, activity lists and medication guides.

There are customized tools for our clients and their caregivers, like daily checklists with boxes for taking meds, checking hearing aid batteries and more. Providers can use it to understand the care needs of the patient, and to document visit notes to ensure continuity of care. And each section includes a signature line for caregivers, so you’ll always know who to call with questions.

We believe the ideal way to connect with clients and their families is in person. However, there are times, when the client and the family live in different areas of the country.
